骨質疏鬆藥反致骨折？

【明報專訊】骨質疏鬆藥本來用以減少骨折的發生，但有病人服用三數年之後，卻出現另一類骨折——大腿骨在沒有撞擊的情況下，無故折斷。本港及美國都有這類個案發生，數目雖不多，但有醫生認為，此現象不能不注意。
骨科醫生指出，讓服藥數年的病人暫時停藥，或可減少這種不尋常的骨折，但在沒有醫生建議前，患者不應貿然停藥。
去年12月的《香港醫學雜誌》及今年3月的《新英格蘭醫學雜誌》，都有醫生報告多宗這種「不尋常骨折」，患者是口服「雙磷酸鹽」類（bisphosphonates）藥物已有數年的骨質疏鬆症患者。
香港大學矯形及創傷外科學系名譽臨脇副教授梁加利指出，這種與骨質疏鬆症患者有關的「不尋常骨折」，已成為醫學會議上的一個課題。
逛街無故骨折
這種骨折如何「不尋常」？梁加利解釋，患者在沒有做任何粗重工夫的情況下，只是在街上逛逛，大腿長骨卻沒來由地折斷。一般來說，大腿長骨只有在遇上很大的撞擊力時——像被車撞或從高處跌下，才會折斷，而且多發生在年輕人身上，但在步行時折斷，則值得關注。
其中《香港醫學雜誌》記載的82歲女患者個案，服用其中一種雙磷酸鹽10年。她的左腿及右腿長骨分別於2003及06年出現「不尋常骨折」，兩次都是在街上步行時發生。
由於如此情況下骨折的當事人是骨質疏鬆症患者，並服用雙磷酸鹽數年（最短的服用時間逾兩年半），醫生便懷疑這種骨折與藥物有關。
3大懷疑原因
基於雙磷酸鹽的特性，他們的懷疑並非沒有道理。
1、骨骼終其一生都在新陳代謝中——造骨細胞製造新骨，破骨細胞淘汰舊骨。雙磷酸鹽的作用，在於抑制破骨細胞的功能，防止舊骨被淘汰，以減慢骨質的新陳代謝速度。梁加利指出，雖然服用者的骨質密度會因而上升，但沒有研究證實它們與一般骨骼一樣堅硬且具彈性。
2、他又稱，骨骼的新陳代謝速度減慢後，內裏的微細損傷不能及時修補。當損傷累積下來，骨骼會變得脆弱，以致「行兩步便斷」。養和醫院內分泌及糖尿病中心主任盧國榮更形容，此時的骨骼，就如疲勞的金屬一樣。
3、骨質疏鬆症患者有三個部位較大機會出現骨折，即手腕骨、脊椎及股骨頸。雙磷酸鹽雖然增加了這三處的骨質密度，減少骨折在這三處出現的機會，但梁加利指出，不排除藥物可能令其他部位的骨骼變得過硬，失去彈性，以致容易折斷。
目前獲准在本港出售的雙磷酸鹽共有三種，出現不尋常骨折的個案，大多發生在服用阿侖膦酸（alendronate）的人身上。香港醫院藥劑師學會藥物教育資源中心教育總監崔俊明指出，這是因為阿侖膦酸是同類口服藥中首種面世且使用時間最長的，故累積的個案也較多。
梁加利稱，服用雙磷酸鹽而出現如此不尋常骨折的個案畢竟是少數，但服用者不能不認識此副作用。由於並非每個服用者都出現這個問題，醫學界正嘗試找出箇中原因。
